- Select Benefits from the menu.

If you’re using the My Canada Life At Work mobile app, it’s a similar journey:

If you’ve got workplace benefits with Canada Life, you can find your coverage details and balances through your My Canada Life at Work account. You can also call us at 1-800-957-9777. If you have Freedom to Choose™ health and dental insurance, you’ll find your coverage information in the product booklet you received when you applied.
While drugs that help manage diabetes, including insulin, are often covered by benefits, plans can vary. You can check your coverage by signing into My Canada Life at Work.
If you’ve got workplace benefits, this depends on your specific plan. You can check your coverage by signing into My Canada Life at Work and searching for the specific vaccine in the Drug search tool. Vaccines aren’t covered by our Freedom to Choose health and dental insurance coverage.
Some prescription drugs require prior authorization before reimbursement is considered. To determine if a drug requires prior authorization, you can:
- Use the drug search tool on My Canada Life at Work
- Call 1-800-957-9777 and speak to a customer service representative
- Ask your pharmacy provider using your drug card. If the drug requires prior authorization, the pharmacy will receive a message advising of the prior-authorization requirement.
Once prior authorization has been confirmed, you must work with your physician to complete and submit a Drug Prior Authorization Form.
If you have coverage through your employer or plan sponsor, use our workplace benefits insurance forms. If you purchased coverage directly through Canada Life, use our Freedom to Choose™ health and dental insurance forms.
When you have filled out the form, ask your doctor to complete the physician information section.
Submit the claim using the instructions on the form.
