Dismemberment benefits

In the event that an employee needs to claim benefits following an accident that resulted in dismemberment, you'll need to provide some info to support their claim.

To do this, you should:

  • Download and save the correct form 
  • Ensure all necessary sections are completed, including the Claimant's Statement where required 
  • Submit the completed form to Canada Life
