Use these forms if you're a beneficiary or estate representative making a life insurance claim on coverage provided through an employer or plan sponsor.

Download the forms and fill them out in Adobe Reader XI or higher (not your browser), and save.

Sign your forms using one of these methods:

• Print the forms and sign them in ink.
• Use a stylus or your finger on a touchscreen.
• Insert an image of your handwritten signature.
• Use a validation system such as Docusign.

We cannot accept forms with a typed signature.

In some cases, you will need to ask the attending physician for a certificate of death. Download this form, and ask them to fill it out.
