Here’s everything you need to know about submitting a claim if you have products through an advisor and what happens after you submit it.
This information applies to the following coverages:

What forms do I need?

You’ll need to complete and submit the appropriate form depending on your claim.

Disability or critical illness claim forms will have 2 parts: Your information and a physician report.

For Freedom to Choose health and dental insurance, make sure you attach all the original receipts that support your claim. We don’t accept photocopies. We won’t be able to return your receipts, but we will provide you with a copy of the explanation of benefits for your records. Learn more about how to process Freedom to Choose claims.

What are the submission timelines?

Submit your claim as soon as possible.

For Freedom to Choose health and dental claims, you must submit your claim within 15 months from the date you received the service or supply.
